The postmortems always blame the same things: the models hallucinate, the data is messy, the team needs more training. What Is Context Fragmentation?
Context fragmentation is the state where the knowledge an enterprise needs to make decisions — definitions, rules, thresholds, relationships, and institutional logic — is scattered across teams, systems, spreadsheets, and individual people's heads with no unified structure.
Every enterprise has a data warehouse. Most have several. These systems are very good at answering one type of question: what happened? Revenue was $42M last quarter. Fill rate dropped to 87%. Twelve SKUs went out of stock in the Southeast region.
But when an AI agent needs to make a decision — should we reallocate inventory? should we accelerate a promotion? should we change the reorder point? — it cannot find the answer in a data warehouse. The Three Failure Modes
Context fragmentation manifests in three specific ways. Every enterprise AI failure we have examined traces back to one or more of these.
1. No Shared Ontology
Ask your supply chain team what "revenue" means. Then ask your sales team. Then ask finance. You will get three different answers. Supply chain might be looking at shipped revenue. Sales is tracking booked revenue. Finance reports recognized revenue. All three call it "revenue" in their dashboards and spreadsheets.
Now build an AI agent that needs to optimize across these functions. Which "revenue" does it use? In practice, it uses whichever data source it was connected to first — and nobody notices the inconsistency until the agent recommends something that makes no sense to two out of three teams.
This is not a data quality problem. The data is accurate. The problem is that there is no shared ontology — no single, authoritative definition of what each concept means, how it is calculated, and how it relates to other concepts in the business.
2. Tribal Knowledge Is Trapped
Your best supply chain planner knows that when lead times from a particular supplier exceed 14 days and demand variance is above 20%, you should bump safety stock by 1.65 times the forecast error. That rule exists nowhere in any system. It lives in her head. When she is on vacation, her backup does not know to apply it. When she leaves the company, that knowledge walks out the door.
Multiply this by every function across the enterprise. Pricing has rules about competitive response thresholds. Trade promotion has heuristics about retailer-specific timing windows. Demand planning adjusts forecasts based on weather patterns that nobody documented. This is the decision logic that actually runs the business — and none of it is accessible to an AI agent.
3. No Decision Layer
Data warehouses store facts. BI tools visualize them. But neither captures the decision logic that connects facts to actions. There is no system that stores: "When this metric crosses this threshold, under these conditions, the correct response is this action, unless these exceptions apply."
This is the missing architecture layer. Without it, every AI agent has to reconstruct decision logic from scratch, usually by asking a human or by hallucinating a reasonable-sounding answer. Neither approach scales.

Case: Fortune 50 CPG Company — The Heatwave Problem
A heatwave hit North India. Within 72 hours, beverage demand spiked 40%. The supply chain team found out about the spike from stockout reports — not from weather forecasts, not from demand signals, not from their planning system. By the time stockout reports surfaced, they had already lost three days of sales in a critical market during peak season.
The weather data existed. The demand elasticity models existed. The supply chain planning tools existed. But no system connected the logic: "When ambient temperature in a region exceeds X degrees for Y consecutive days, beverage demand in that region will increase by Z%, and the supply chain response should be to pre-position inventory from warehouses A and B." That decision logic lived in one regional manager's experience. He was on leave that week.
Case: Healthcare Company — The Contradictory Truth
Nielsen syndicated data showed market share was down 1.2 points. Internal sales data showed billing was up 8% year-over-year. The executive team spent three weeks trying to figure out which number was wrong. Neither was.
Nielsen was measuring share of a category that had expanded by 15%. Internal billing was measuring absolute revenue against prior year. Both were accurate descriptions of reality from different vantage points. But no system could explain why these numbers appeared contradictory, because no system stored the ontological relationship between market share (a relative metric) and billing revenue (an absolute metric) in the context of category expansion.
An AI agent asked to "explain our market performance" would have picked one data source and given a confident, half-true answer.
Why "More Data" Is the Wrong Answer
The instinctive response to context fragmentation is to throw more data at the problem. Build a bigger data lake. Connect more sources. Add a semantic layer. Fine-tune the model on more enterprise documents.
This does not work, and understanding why is critical.
More data gives agents more facts. But facts without decision logic produce the same failure at larger scale. You go from an agent that does not know which "revenue" to use, to an agent that has access to fifteen definitions of "revenue" and still does not know which one applies in which context.
The problem is not the volume of data. It is the absence of structured context — the ontology, the rules, the relationships, and the decision logic that make raw data actionable.
Consider the difference:
- "More data" approach: Give the agent access to weather APIs, demand forecasts, inventory systems, and supply chain planning tools. Hope it figures out the connections.
- "Structured context" approach: Encode the relationship between weather patterns and demand elasticity, link it to inventory thresholds and supply chain response protocols, and give the agent a traversable graph of decision logic to follow.
The first approach produces impressive demos. The second approach produces production systems.

Cross-functional traversal: When a supply chain event happens, can your systems automatically trace the impact through pricing, promotion, and sales? Or does that require a human to connect the dots?
Scoring: If your total is below 6 out of 12, your AI agents are building on fragmented context. The model is not the bottleneck. The infrastructure is.
